1/17/2023 EnchantaVaughn Approved Once the session began, client was attentive and focused and displayed a high level of confidence when reflecting on her feelings. Client shows positivity to authority by giving them the same respect they give her, while utilizing proper voice volume in the home. Client was careful not to get off task, as seen listening to instructions carefully. Client was observed following directions. Client was friendly, invited mom to the discussion and wanting her advice when it came to other things that she was afraid to discuss alone with her counselor at the time, including but not limited to her hygiene, home duties, and mental health treatment plan. Client was in a calm mood, as evidenced by her relaxed facial expressions and demeanor. Client was seen holding a conversation with her family and displaying appropriate manners. Client was observed showing good leadership skills as evidenced by her courage and sportsmanship with speaking in front of others. Client expressed a positive conversation with her counselor and maintained excellent voice volume in the home. Client was seen following directions and completing the worksheet activity independently. Client required little help following the assignment and showed she could be a leader and take initiative of her own self and be responsible. QMHP collaborated with mom in regard to home rules and regulations, behavior management, effective teaching practices, following verbal and non-verbal instruction, role playing exercises, and an appropriate incentive system. QMHP educated client about using good manners at home and offering alternative methods and suggestions to model appropriate language that client should use to ask for things or to get someone’s attention. QMHP processed client feelings and offered choices of what to do when faced with conflict or challenged in the home setting. QMHP facilitated a family discussion to talk about common mistakes children with ADHD make and how she can make better choices. QMHP reviewed home expectations and how client should conduct herself around peers and authority. QMHP encouraged client to continue displaying age appropriate behavior, and the benefits of this will be reviewed with parent following each session. QMHP conducted a review of the previous week and asked client what she learned from it. QMHP assisted client with problem-solving skills. QMHP provided client with positive feedback due to her responses. Client engaged in self-reflection and reflected on her day, the good and the bad in front of mom and seemed happier and more open minded with the interventions that were displayed. Client says please, thank you, and excuse me appropriately during the session. Client shows empathy towards mom, as evidenced by her hugging, playing, smiling, and complimenting her. Client avoids negative language (i.e. profanity words). Client recognized her triggers and was able to identify common interests and similarities she and other ADHD recipients shared. Client understand that she is expected to act civilized and use her inside voice while inside and transition safely to session and to places in the community that are safe and therapeutic. Client is observed making better choices and doing the right thing at home. Client has improved with staying seated without fidgeting behavior and utilized her breaks appropriately without displaying attention-seeking behaviors to get her way. Client responds well to praise and support. Client is acceptive of redirection. Client understands that her actions have consequences. Client is striving for a good report from mom and family every day.
